All new patients should complete the Elements Intake form above on their first visit to our clinic, along with the specific form(s) below for the service(s) you are seeking. If you have an appointment with our mental health team, your forms will be emailed to you separately.

Direct Billing
We direct bill for all eligible services. Please bring your insurance company information with you to your first visit. To save time, please complete your appropriate direct billing agreement(s) before you come. For Sunlife or benefits ran through Telus Health (click here to see if your insurer uses Telus Health) please complete the appropriate form below.
